Billing and Insurance Information

HEALTH INSURANCE PLANS

Our office staff will assist you with determining which of our providers are contracted with your particular insurance policy. To help ensure converage, please contact your insurance to obtain benefit information or authorization for a particular service.

Please be aware that copayments and/or deductibles are often associated with insurance plans, for which you could be responsible.

BILLING OPTIONS

Your services may be billed through your insurance policy or, in some cases, you may choose to arrange a private pay arrangement with your therapist. Read more about our Good Faith Estimates here.

PAYMENTS

ALl payments are due at the time of service. Payments include but are not limited to: private pray agreements, office copayments and insurance deductible payments. Clients are required to keep a credit card on file.

CHANGES TO INSURANCE

Please notify the office staff if you anticipate any changes to your insurance plan. If you receive a new card, please bring it to your next appointment or call the office even if you believe the information is the same.
